What You Can Expect
· Lead Patient-Centric Care: Provide direct, compassionate patient care as part of a multidisciplinary team ensuring every patient feels supported and understood.
· Tailor Medication Therapy: Take the reins in initiating, adjusting, and discontinuing medications, crafting personalized treatment plans that optimize outcomes for each patient.
· Proactive Monitoring: Order and monitor labs while performing limited physical assessments, keeping a close eye on patient progress and making timely adjustments as needed.
· Chronic Condition Management: Become a key player in managing obesity and guiding patients to achieve their health goals.
· Educate and Empower: Share your expertise through drug information and education for healthcare providers, ensuring they have the knowledge to support their patients effectively.
· Collaborate Across Disciplines: Engage in shared visits with other providers, fostering a holistic approach to patient care that integrates multiple perspectives and expertise.
· Enhance Patient Understanding: Conduct comprehensive medication education and provide engaging patient education, empowering individuals to take charge of their health journeys.

Qualifications (Update to the standard language)
· Idaho Pharmacist License
· PharmD
· BLS
Preferred Qualifications
· Experience in independently managing chronic diseases.
· Completion of PGY2 Ambulatory Care Residency OR completion of a PGY1 pharmacy residency program with ambulatory-focus OR equivalent ambulatory care experience.
· Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or similar.
