Nurse Liaisons are a critical role for the success of all post-acute care. A successful liaison will have great people skills, be able to build trusting relationships with hospital staff and providers and have a true passion and desire to care for end-of-life patients to ensure their needs are met. You will coordinate care within the Treasure Valley area.
What you can expect from this role:
The Post-Acute Liaison for Hospice is a critical role focused on facilitating seamless transitions for patients into hospice care at the appropriate time. This position involves collaborating with post-acute care teams, processing referrals from acute care settings, evaluating patient eligibility for hospice services, and coordinating care to ensure patients reach their preferred place of care, whether at home or in a facility. The Liaison builds trusted relationships with case managers, healthcare providers, and community stakeholders to support patients and families during critical moments, particularly those with conditions such as cancer, neurological disorders, or heart failure, ensuring they receive compassionate, high-quality care in the optimal setting.
