Overview

St. Luke’s Health System is seeking a Telehealth Patient Management Specialist to join our Ambulatory Care team. In this role, you will help support patients with chronic conditions. You will be an essential part of a collaborative team focused on improving access to care, reducing hospital visits, and empowering patients to manage their health more effectively.

We currently have both full-time and part-time onsite positions available.

 

Position Details:

  • Shift: 3x12-hour shifts (7:00 AM – 7:00 PM)

  • Schedule: Rotating weekends and holidays required

  • Department: Telehealth – Ambulatory Remote Patient Monitoring

What You’ll Do:

  • Onboard new patients into the remote patient management program

  • Monitor and manage patients living with chronic conditions

  • Assist patients in adhering to care plans and maintaining compliance with program guidelines

  • Provide basic technical support for remote monitoring equipment

  • Perform scheduling functions, similar to a clinic setting

  • Utilize medical terminology and clinical knowledge to support communication between patients and care teams

Qualifications:

High school diploma or equivalent required

 

Preferred Qualifications:
- Experience as a Certified Nursing Assistant (CNA), Unit Clerk, or in a similar role
- Familiarity with medical terminology
- Background in a healthcare setting, ideally ambulatory or telehealth
