What Makes This Role Exciting:
As an Ambulatory Care Heart Failure Pharmacist embedded in our cardiology clinic, you will play a key role in optimizing therapy for patients with complex heart failure. This is an opportunity to support cardiologists and cardiology APPs in delivering advanced care and to lead efforts in medication management within a multidisciplinary team. What You Can Expect

  • Optimize Heart Failure Therapy: Independently initiate, adjust, and discontinue guideline-directed medical therapy (GDMT) and diuretics for patients with heart failure.
  • Support Advanced Therapies: Manage specialized medications including GLP-1 receptor agonists, Camzyos, Amvuttra, and infusion therapies.
  • Collaborate Closely: Partner with cardiologists, APPs, nurses, and care coordinators to drive outcomes and ensure safe, efficient care.
  • Lead Patient-Centric Care: Provide independent, face-to-face clinical visits focused on assessment, education, and medication management.
  • Monitor and Adjust: Track patient progress, labs, and symptoms to make timely and evidence-based medication changes.
  • Navigate Complexity: Work with patients who may have LVADs, be awaiting transplant, or participating in clinical trials.
  • Empower Patients and Teams: Provide medication education to patients and serve as a clinical resource to your interdisciplinary team.

Qualifications:

  • PharmD
  • Idaho Pharmacist License
  • BLS
  • Current Board Certification in Cardiology Pharmacy (BCCP) or eligibility within 2 years of starting

 

Preferred Qualifications

  • Previous clinical management of heart failure patients.
  • Completion of PGY2 Cardiology Residency OR completion of a PGY1 pharmacy residency program with a cardiology-focus OR equivalent heart failure disease experience.
