The Hospice Supervisor coordinates the integration of Care/Case Management functions to provide efficient and effective outcome-based, patient-centered care.  You will provide oversight of Hospice staff which includes leading, educating and assessing performance of staff.

 

What You Can Expect:

  • Full Time Mon-Fri from 8:30am-5:00pm (rotates through administrative call)
  • Be a part of a collaborative and supportive team
  • Develop and mentor others
  • Serve as expert and resource to staff
  • Work collaboratively to improve outcomes

Qualifications:

    • Education: Bachelor's Degree in Nursing
    • Experience: 3 years of relevant experience, preferably in Hospice setting
    • Licenses/Certifications: Current RN licensure in State of Practice; current Basic Life Support (BLS) Provider Certified through American Heart Association or American Red Cross
