Medication Management Pharmacist – Anticoagulation / GLP-1 / Emerging Disease States

Location: Twin Falls, Idaho
Organization: St. Luke’s Health System
Schedule: Monday–Friday, 8:00 a.m. – 5:00 p.m.

Position Summary

The Medication Management Pharmacist is responsible for the delivery of high-quality pharmaceutical care to patients and customers of St. Luke’s Health System. This role will primarily support medication management services in anticoagulation and GLP-1 therapy, with opportunities to expand into additional disease states as services evolve. The pharmacist works collaboratively with providers, nurses, and other healthcare professionals to optimize medication therapy outcomes in accordance with medical center standards.

Key Responsibilities

  • Review and assess the appropriateness of prescribed medication therapy to ensure safety, efficacy, and adherence to evidence-based guidelines.
  • Provide medication information, education, and consultation to patients, nurses, and medical staff.
  • Interpret prescriptions and accurately compound and dispense medications in compliance with state and federal regulations.
  • Perform cognitive clinical pharmacy services and document findings, interventions, and actions in accordance with medical center standards.
  • Administer medications and vaccinations as appropriate and within scope of practice.
  • Supervise, authorize, and support the work of pharmacy technicians, externs, interns, and other support staff.
  • Prepare and maintain accurate records related to medication therapy, prescriptions, patient charges, medical records, and inventory.
  • Assist in the training and development of technicians, externs, interns, residents, and other department staff as needed.
  • Serve as a role model and mentor to technicians, students, residents, and new practitioners.
  • Perform other duties and responsibilities as assigned to support departmental and organizational goals.

Qualifications

Education

  • Degree from an accredited pharmacy program (PharmD or equivalent).

Experience

  • No prior experience required.

Licenses and Certifications

  • Valid Pharmacist License in the state of practice.
  • Current Basic Life Support (BLS) Provider certification through one of the following:
    • American Safety and Health Institute
    • American Heart Association
    • American Red Cross
