  • Overview

    We are looking for a Registered Nurse- Case Manager to join us in our strategy to be the community’s trusted partner in providing exceptional, patient-centered care. The Registered Nurse- Case Manager assists patients to reach optimal function and health in an appropriate environment that would support their needs. The Case Manager functions within the full scope of their practice to include duties and responsibilities of the position description and to extend these responsibilities beyond the episode of care to encompass coordination of care across the continuum. The length of time a Case Manager would follow a patient is determined by patient needs from admission through discharge from post-acute care facility. The Case Manager is responsible for developing collaborative relationships internally and externally, with contracting and non-contracting facilities ensuring they meet the Triple Aim and criteria set by the Select Medical Network or St Luke's Health System. This role will float between Boise and Meridian sites.

    Required Criteria 

    • Degree in Nursing from an accredited school of nursing required.
    • Current Idaho nursing licensure required.
    • A minimum of two years of RN experience.


    Preferred Criteria

    • Preferred Certification in Case Management (CCM) through an accredited program.
    • Focus of care management from an outpatient perspective with proficiency in evaluating current and ongoing physical, psycho-social and functional needs required.
