St. Luke’s Health System in Fruitland is seeking a Case Manager to join our Social Services team.


**Swing, 12 hour shifts


  • Works as consultant, educator, or community liaison to facilitate collaborative coordination of patient care.
  • Uses assessment skills and risk assessment tools to identify patients with actual or potential health care needs that would require transitions of care coordination.
  • Collaborates with patient/family and interdisciplinary team in establishing mutual goals based on the patient’s needs or problems.
  • Coordinates care across the continuum (inpatient/outpatient/community) to assure appropriate utilization of clinical and community resources.
  • Applies specialized expertise to solve problems, generate ideas, plan care, and teach.
  • Evaluates patient's progress towards identified goals.
  • Assists the care team by identifying potential barriers that impedes access to care. Implementing plan to address barriers.
  • Coordination of primary care and sub-specialty providers.
  • Serves as subject matter expert within specialty area.
  • Perform other duties and responsibilities as assigned.


  • Education: Master's Degree in Social Work.
  • Experience: None
  • Licenses/Certifications: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of practice.
