St. Luke’s Health System in McCall is seeking a Part Time Social Worker to join our Home Care and Hospice team.
**This position will work 24 ours per week and includes benefits!
Home Health will provide a wealth of experience and knowledge of the “skilled” Home Health and provision of care within the community we will provide an opportunity for professional growth and development.
Our Home Health and Hospice is a highly regulated area of care as we are providing care within the home setting which takes individuals who are comfortable being independent and autonomous but while also working very closely and collaboratively with the interdisciplinary team of other skilled professionals of Nurses, Physical Therapists, Occupational Therapists, Speech Language therapists and Respiratory Therapists, Nurse Practitioners.
This role is a blend of clinical interventions, care plan development, gaining knowledge of Social Security, Medicare/Medicaid, Indigent Resources. The MSWs are actively involved in advance care planning, addressing adjustment to illness whether acute/chronic, grief/loss and end of life discussions along with ongoing networking with community resources ranging from behavioral health, facility placements to name a few.
Home Health provides care for patients of all ages ranging from pediatrics to geriatrics that are typically home bound and require care that can not be met by family member or a in-home caregiver.
