St. Luke's is seeking an Ambulatory Care Pharmacist to join our team for our downtown Boise Family Medicine Clinic.

 

What you can expect

  • Workunder a broad collaborative practice agreement to provide chronic disease state medication management
  • Manage a fast-paced clinic schedule of 8-12 patient appointments per day
  • Provide support to providers and patients through answering drug information questions, trouble-shooting pharmacy problems and educating on medications
  • Coordinate with a multidisciplinary team of physicians, advanced practice practitioners, behavioral health specialists and care managers
  • Collaborate with clinical pharmacy technicians
  • Opportunities for precepting pharmacy students and residents

Qualifications

  • Degree from an accredited pharmacy program
  • Valid Idaho Pharmacist License

Preferred Qualifications

  • Previous experience with the clinical management of primary care based chronic diseases
  • Completion of PGY2 Ambulatory Care OR completion of a PGY1 pharmacy residency program with ambulatory care-focus OR equivalent ambulatory care experience
  • Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
