We are seeking a Pharmacist to join our Ambulatory Pharmacy team. Unique in nature, this pharmacist will join a newly created, multi-disciplinary team supporting our Primary Care South Nampa Clinic virtually from a clinic in Boise. In this position, you are an essential team member of an outpatient healthcare team improving quality and patient outcomes by focusing on medication management.

 

What you can expect

  • Serve as a valued member of our integrated, accessible health care services team, addressing medication needs and developing sustained partnerships with patients
  • Tap into consumer insights to shape the care environment
  • Coordinate with a multidisciplinary team of physicians, advanced practice practitioners, behavioral health specialists and care managers, using a flexible mindset to adapt to consumer needs and preferences
  • Serve pediatric, adult, and senior patient populations, addressing new health problems, chronic diseases, preventative care, and acute care not requiring an emergency room visit
  • Coordinate with the pharmacy team to capture 340b medication referrals through pharmacotherapy/MTM services

Qualifications

  • Degree from an accredited pharmacy program
  • Valid Idaho Pharmacist License

Preferred Qualifications

  • Clinical management of primary care based chronic diseases
  • Completion of PGY2 Ambulatory Care Residency OR completion of a PGY1 pharmacy residency program with ambulatory-focus OR equivalent ambulatory care experience
  • Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
