Overview

St. Luke's Health System in Boise, ID is seeking a Network Engagement Specialist to join our Health Partners team.

 

Under general direction, the Network Engagement Specialist facilitates practice engagement around St. Luke's Health Partners' (the Network) programs and initiatives to support patient preventative care visits, screenings, and quality measure activities managed by the Network's Population Health Team (PHT). This role is responsible for outreach to the Network's value-based contract (VBC) members to aid them in matching with a network provider and assisting them with the appropriate entry point for establishment of care, taking into consideration any available information about geographic location, access to care, unique social determinants of health (SDoH), and health literacy.

 

  • Utilizes clinical or claim-based reports to make outbound patient phone calls to assist practices (independent or employed) in scheduling patients for appropriate preventative screenings, annual wellness, or care visits to support quality improvement and clinical documentation/discernment initiatives.
  • Reviews and distributes reports, coordinating patient appointments with primary care or specialty practices as appropriate, locating and obtaining outside patient records as needed through Electronic Health Record (EHR)/Health Information Exchange (HIE) access or otherwise, and other critical tasks to support programs such as Medicaid Value Care Organization (VCO), Medicare Shared Savings Program/Accountable Care Organization (MSSP/ACO), the St. Luke's Employee Health Plan, or other advanced payment methodology (APM)/VBC contracts.
  • Using standardized tools and scripting, perform outgoing patient phone calls aimed to screen, educate, motivate, and schedule patients for preventive screenings, annual wellness visits, and in-home test kit follow-up visits.
  • Performs proactive outreach to unassigned St. Luke's Health Partners (SLHP) VBC contract members to assist them in establishing care with a Primary Care Provider (PCP) who meets their care needs and assists with visit scheduling as needed.
  • Responsible for managing incoming provider or patient phone calls, appropriately triaging patient questions and needs to SLHP staff, St. Luke's Clinics, or independent participating practices.
  • Reviews patient lists for prioritization of outreach, necessity of gap closure, and chronic condition recapture. Engages practices as appropriate around this work.
  • Locates patient clinical records to support pertinent gap closure information and entry into payer portals as needed. Supports patient PCP changes in payer portals as appropriate.
  • Coordinates incoming and outgoing information for the SLHP PHT and other various administrative tasks needed to support the team in the areas of contract performance, quality improvement initiatives, and clinical documentation/discernment enhancement.
  • Performs other duties and responsibilities as assigned.

 

Minimum Qualifications:

Education: Associate's degree or experience in lieu of degree

Experience: 2 years relevant experience
