What you can expect from this role
You will provide PRN, admission/informational, imminent and some routine visits for terminally ill patients in homes. You will collaborate with the IDT team regarding patient's care plan to provide symptom management care, emotional and spiritual support, medication management, and education. This person needs to be able to work autonomously and as a team, have interpersonal and communication skills, and have a true passion and desire to care for end-of-life patients to ensure their needs are met.
