What You Can Expect:

  • Support both Home Health and Hospice
  • Completes psychosocial assessments and plans of care for home health patients.
  • Identify and provide interventions and goals as indicated by the plan of care.
  • Consults with physicians, healthcare professionals, the home health care team, and community partners.
  • Part of the multi-disciplinary team.
  • Provides community resource information to home health patients/families.
  • Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling or a fleet car
  • Bonuses: Sign On bonus eligible
  • Professional Development: Clinical Supervision and in house educational opportunities
  • Schedule: Monday-Friday schedule

 

Qualifications:

  • Education: Masters degree
  • Licenses/Certifications: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in states of practice
  • Other Requirements: Valid driver’s license and proof of auto insurance
