With opportunities for growth, competitive benefits, and a supportive community of colleagues, St. Luke’s is truly a great place to work.   We are seeking an Infectious Disease Clinical Pharmacy Specialist – Outpatient Parenteral Antimicrobial Therapy (OPAT).    This OPAT Specialist will implement a NEW PRACTICE in collaboration with our infectious diseases and antimicrobial stewardship teams.   What you can expect - Address pharmacy-related questions from clinic providers and manage infectious diseases under collaborative practice agreement - Collaborate as a a member of the infectious disease (ID) clinic team and work closely with physicians and APPs to ensure appropriate long-term antimicrobials and monitoring - Assist with determination of OPAT candidacy and regimen selection - Counsel patients on OPAT regimens - Oversee pharmacotherapy monitoring for patients on OPAT service - Verify, order, and review medications and labs for OPAT - Manage arising symptoms or lab values and change antimicrobials or follow up plan as needed - Determine if/which IV access is appropriate - Order IV access removal or otherwise verify removal - Support the System Antimicrobial Stewardship Team and Antimicrobial subcommittee - Collaborate with System ID pharmacist, Antimicrobial Stewardship Team, physicians, and colleagues to ensure smooth hand off for transition to outpatient pharmacy - Collaborate with Infectious Diseases/Antimicrobial Stewardship Program Coordinator on quality improvement initiatives - Support transitions of care efforts   Qualifications - Degree from an accredited pharmacy program required - Valid Idaho Pharmacist License Preferred qualifications - Primary care-based ID clinical management experience - PGY2 Infectious Diseases Residency OR equivalent experience - Current Board Certification in Infectious Diseases Pharmacotherapy (BCIDP)
