The SLCI RN is the keystone to the complex, high-quality, compassionate care we provide to the communities we serve.   What you can expect from this role   The Clinical Oncology RN is responsible for prioritizing walk-in and telephone triage encounters to address presenting symptoms with focused assessments, management, patient teaching, and coordination of patient care needs, in consultation with providers, key team members, and outside health associates, i.e., insurance, pharmacies, etc. who are involved with the patient's plan of care. Embedded within a large family medicine practice in Twin Falls, this pharmacist will work between the Addison Family Medicine Clinic and the Medical Plaza Family Medicine Clinic. This role is a vital part of the health care team within our primary care clinics!   What you can expect - Work under a broad collaborative practice agreement to provide chronic disease state medication management - Manage a fast-paced clinic schedule of 8-12 patient appointments per day - Provide support to providers and patients through answering drug information questions, trouble-shooting pharmacy problems and educating on medications - Coordinate with the multidisciplinary team of physicians, advanced practice practitioners, behavioral health specialists and care managers - Collaborate with clinical pharmacy technicians - Opportunities for precepting pharmacy students and residents Qualifications - Degree from an accredited pharmacy program - Valid Idaho Pharmacist License Preferred Qualifications - Previous experience with the clinical management of primary care based chronic diseases - Completion of PGY2 Ambulatory Care OR completion of a PGY1 pharmacy residency program with ambulatory care-focus OR equivalent ambulatory care experience - Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
