Clinic services include electrophysiology and arrhythmia exams and consultations, stress tests, and nuclear cardiology.   The RN Nursing Supervisor duties consist of administration/supervisory as well as serving as a clinic RN. St. Luke’s Health System in McCall, ID is seeking a Hospice Registered Nurse (RN) Coordinator to join our Hospice team.   The St. Luke's Health System Hospice team is looking for a Hospice Registered Nurse (RN) Care Coordinator/Case Manager who will manage their own caseload of patients in the community, with a scope of practice to include symptom management, medication management, care planning and IDT oversight.   Hours are between:  Monday - Friday (8:30am - 5:00pm) with rotating weekends and evening call.   Paid hourly, manageable caseload, pay for drive time, mileage reimbursement and work/life balance!   St Luke's Hospice is an interdisciplinary team serving terminally ill patients in homes, ALFs, CFHs, and SNFs with diagnosis mix of Cancer, Heart Disease, Pulmonary Disease, Alzheimer’s/Dementia, Renal Disease, ALS, Liver Disease, Stroke, etc. Team members work autonomously in the community setting serving patients within the spectrum of the 4 levels of hospice care: Routine, Continuous, General Inpatient, and Respite.   The interdisciplinary team is comprised of Care Coordinator RNs, Weekday Support RNs/LPNs, Weekend Support RNs/LPNs, Night Support RNs, Medical Social Workers, Chaplains, Certified Nursing Assistants, Massage therapists, Volunteers, and Medical Staff including a nurse practitioner and physicians.   Strong team communication with individual autonomy is required. Dependable, strong critical thinking, and a self-starter are preferred qualities in candidates. Strong nursing comprehension of disease processes is essential. Collection of data is via interview, observation, auscultation and palpation. Through documentation and oral communication, essential information is shared with health care team members and other customers common to the unit/area(s) of assignment. Clinic Hours: - The majority of clinics are open from 0800-1730 Monday-Friday - We have full and part time positions available About the Ambulatory Clinic Ahead Program (ACAP): The Ambulatory Clinic Ahead Program is an exciting opportunity for LPNs to receive standardized training and the opportunity to rotate through clinics to find permanent placement in a Treasure Valley clinic that is an ideal fit. Your Treasure Valley work location will vary based on clinic staffing needs and preference. - Opportunity to start in a float pool position and experience a variety of clinics - Family medicine, internal medicine, and Specialty clinics (Neurology, Pediatrics, Cardiology, Orthopedics, etc.) - Get to know the staff, clinic flow, and schedule prior to accepting permanent position - Think of this as a “working interview” with your potential clinic Responsibilities - Provide and document direct and individualized nursing care to patients based on the application of scientific nursing principles. - Perform medical procedures within scope. - Assists physicians and performs healthcare services to patients in hospitals, private homes, clinics, and other healthcare facilities. - Identifies and reports patient care needs through data collection, planning, intervention and evaluation. - Identifies patient need for education and provides approved hospital education materials. - Perform other duties and responsibilities as assigned.
