You will provide comprehensive care to patients with a new diagnosis, medications, or have been discharged from ER, require respiratory treatment, and wound care. During home visits, you will ensure patients are progressing with their care plan, provide education related to their disease process, and assess physical and psychological needs to improve quality of life. When it comes time to discharge, you will coordinate resources and educate patient and patient's family.

Qualifications
- Education: Nursing Degree
- Experience: 1 year of RN experience; Hospice or Acute Care experience helpful
- Licenses/Certifications: Current RN License in the State of Practice. Current Basic Life Support (BLS) Certification Provider Certified through American Heart Association or American Red Cross
- Valid Driver's License and proof of Auto Insurance
