Working with the patient to educate and make sure they understand their health needs, discharge medications, upcoming appointments and educating them on infection prevention at home.

Schedule
Could be Monday - Friday, 8am - 5pm, or possibility of four 10-hour shifts. This position is extremely independent after training and has a team to partner in patient goals to help contribute to your success in this position. Must be able to manage and organize their own day to include quality audits and chart review. Location is primarily in our Boise location off 6th and Franklin with a chance to work remote from home in the Boise Metro area.
