The Home Care Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care from admission to discharge for an assigned case load. This is a direct care position providing in home care that requires traveling within the Treasure Valley area.

Position FTE: .4 FTE (32 hours per pay period) - Evening Shift. Shifts can start any time from 0900 to 1300 and end as late as 2330, Monday-Friday.
The PACU RN department has a variety of 8, 9, and 10 hour shifts to meet employees FTE requirement. Unit Summary
Our unit is composed of some of the best nurses in Southern Idaho. This is a blend unit of periopertive sevices. The department cares for patients getting ready for and recovering from surgical procedures, it includes pre-op, PACU and SDS. The department requires that you be cross trained to two subunits, critical care experience is preferred to work in the PACU. We prepare and recover patients that require surgical interventions for service lines such as vascular, urology, ENT, general surgery, orthopedics, plastics, gym, gastroenterology, and podiatry. Unit/Position Summary
- 6 East is a fast-paced 35 bed Surgical unit that is an ideal place to build strong clinical skills while caring for an increasingly diverse patient population.
- Typical RN to patient ratios on this unit are 1:5, with reduced ratios of 1:4 for acuity when necessary. For CNAs, the ratios are typically 1:8-10.
The Registered Nurse is responsible for:
- Evaluating patients and assisting physicians with patient care.
- Plans, implements, and documents nursing care based on patient’s needs.

Meridian Med/Surg 5th and 6th floor cares for patients with acute medical, surgical, and minor cardiac and respiratory conditions. It is a dynamic environment rich with opportunities for learning. Staff is very collaborative and energetic and believes in team work to reach the best patient outcome. The unit is designed to provide high quality, personalized care for patients. We use a multidisciplinary approach to patient care that is patient centered and encourages teamwork between nursing staff, physicians, therapy teams, social work, care coordination, and pharmacists. Opportunities to get med/surg, pain management, ortho, and/or neuro certified. Units 5 and 6 are 42 bed units and have an average nurse/patient ratio of 1:5.

The average nurse/patient ratio is 1:3-4. 9 East is an action-packed work environment known for its teamwork and passion for patient care. We are a Joint Commission Certified Spine Program of Excellence and Certified Primary Stroke Center of Excellence.
