This position will be a part of our Pharmacotherapy Pharmacist team in Twin Falls.  This role is a vital part of the health care team within our primary care and cardiology service line!   What you can expect - Work in a pharmacist-led outpatient pharmacotherapy clinic, which accepts referrals for anticoagulation management AND primary care chronic disease state management - Work under a broad collaborative practice agreement to provide medication management - Manage a fast-paced clinic schedule - Provide support to providers and patients through answering drug information questions, trouble-shooting pharmacy problems and educating on medications - Coordinate with the multidisciplinary team of physicians, advanced practice practitioners, behavioral health specialists and care managers - Collaborate with clinical pharmacy technicians - Opportunities for precepting pharmacy students and residents Qualifications  - Degree from an accredited pharmacy program - Valid Idaho Pharmacist License Preferred Qualifications - Previous experience with the clinical management of anticoagulation, hypertension, diabetes and other chronic disease states - Completion of PGY2 Ambulatory Care OR completion of a PGY1 pharmacy residency program with ambulatory care-focus OR equivalent ambulatory care experience - Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), Geriatric Pharmacy (BCGP) or Cardiology (BCCP)
